چکیده
Objective To assess improvement in autonomic symptoms and functional impairment following immunotherapy with subcutaneous immunoglobulin (SCIG) or plasmapheresis (PLEX) patients postural orthostatic tachycardia syndrome (POTS). Background POTS is a common disorder defined by an increased heart rate of at least 30 bpm within 10 minutes standing tilt table test, accompanied intolerance, fatigue, dizziness, headache. Despite pharmacologic non-pharmacologic therapy, the marked associated reflects great need for improved treatment. Autoimmunity has emerged as leading etiology POTS, case series describing successful treatment IVIG. our knowledge, SCIG not been described previously. Design/Methods Clinical history seven treated PLEX was reviewed. Response to assessed using COMPASS-31 ability scale (FAS) completed retrospectively pre- 3-12 months post-treatment PLEX. Patients comorbid autoimmune disorders immune deficiency requiring were excluded from study. Results Of patients, all female, ages 28-57, five received two Four had small fiber neuropathy various positive antibodies low titers. Across FAS scores average 50% 217%, respectively. Six able discontinue reduce oral medications reported being return work school. No serious adverse events reported. Conclusions experienced significant reduction This suggests that may be safe effective treatments severe refractory standard therapies. Randomized controlled trials are needed determine efficacy safety these long-term

Postural tachycardia syndrome (POTS).
INTRODUCTION POTS is defined as the development of orthostatic symptoms associated with a heart rate (HR) increment >or=30, usually to >or=120 bpm without orthostatic hypotension. Symptoms of orthostatic intolerance are those due to brain hypoperfusion and those due to sympathetic overaction. METHODS We provide a review of POTS based primarily on work from the Mayo Clinic. RESULTS Females p...

Orthostatic intolerance can be defi ned as inability to tolerate upright posture relieved by recumbence. Postural orthostatic tachycardia syndrome (POTS) is a form of orthostatic intolerance defi ned as sustained increase in heart rate of ≥30 bpm or increase of heart rate to ≥120 bpm within 10 min of standing or head-up tilt associated with symptoms of orthostatic intolerance and absence of ort...
متن کاملOrthostatic intolerance: postural orthostatic tachycardia syndrome with overlapping vasovagal syncope.
A 28-year-old female with a history of situational syncope and a new-onset right sided hemiparesis is described. Tilt-up table test revealed the postural orthostatic tachycardia syndrome followed by vasovagal syncope. Neurological and internal medicine tests showed no particular disorders. The patient underwent autonomic physical training and the tilt-up test performed three months later showed...
متن کاملPostural orthostatic tachycardia syndrome (POTS) and vitamin B12 deficiency in adolescents.
OBJECTIVE Vitamin B12 is involved in the production of adrenaline from noradrenaline. It is the cofactor involved in catecholamine degradation and plays a role in myelin synthesis. The current study aimed to investigate the association between vitamin B12 levels and postural orthostatic tachycardia syndrome (POTS) during adolescence when accelerated myelin synthesis increases the vitamin B12 ne...
